ASSESSMENTS & REPORTS

HOW WE CAN ASSIST YOU

Navigating the NDIS funding process, plan reviews, and eligibility requirements can feel overwhelming. As Occupational Therapists, we are here to support you.

We specialise in translating your real-world needs into the precise, clinical language the NDIS requires without losing sight of who you are. We provide rigorous, high-quality Occupational Therapy assessments and reports that serve as powerful advocacy tools.

CORE ASSESSMENT SERVICES

We provide a comprehensive suite of professional Occupational Therapy reports designed specifically for children to adults (aged 2-35).

functional capacity assessment

Initial Assessment

An Initial Screening Assessment is required before therapy begins (except for FCA or SIL-only services). Uses observations and standardised tools to evaluate sensory processing, motor skills, social interaction, and self-care, providing a baseline for therapy goals and recommendations in a summary report.

FCA

Handwriting

Evaluates a person's fine motor skills, visual-motor integration, and pencil grip to identify underlying barriers to written communication. Based on these findings, an occupational therapist develops tailored strategies and exercises to improve legibility, speed, and writing comfort.

SIL housing

Supported Independent Living

This NDIS-required evaluation determines funding levels by identifying the what support  participant needs to live safely and independently.

Supported independent living

Sensory Processing

Evaluates how an individual perceives, interprets, and responds to sensory information from their environment and body. These insights allow an occupational therapist to design targeted regulation strategies, modify spaces and improve comfort in daily activities.

Functional Capacity Assessment

A Functional Capacity Assessment (FCA) is an NDIS-required occupational therapy evaluation that identifies the physical or supervisory support a participant needs to live safely and independently, providing key evidence for funding.

Motor Skill Coordination

Evaluates an individual's gross and fine motor abilities, focusing on balance, dexterity, and bilateral coordination. These insights allow an occupational therapist to develop targeted interventions that enhance physical control, confidence, and independence in everyday tasks.

THE ASSESSMENT PROCESS

Here is how our OT team makes the assessment and reporting process as smooth and supportive as possible:

Step 01:

PARENT/CAREGIVER INITIAL CONSULTATION

🚀 Parent Only

🚀 Discuss developmental history, goals, and current support needs

🚀 Waiting to meet the participant till the second session allows for conversation to be had without little ears around

🚀 Can be telehealth

Step 02:
CONNECTION & RAPPORT BUILDING

🚀 The first session with the participant is dedicated to building rapport and establishing a sense of safety

🚀 Formal testing may or may not begin immediately

🚀 The " settling in" period is vital, as it allows us to gather essential observations

Step 03:
REAL-WORLD OBSERVATION & ASSESSMENT

🚀 Once rapport is established, conduct formal and informal assessments

🚀 Ensures the data we collect reflects true functional capacity rather than "performance"

Step 04:

COLLABORATIVE GOAL SETTING

🚀 We work with families to ensure our clinical findings align with their and their families' goals.

🚀 Set long and short term goals that direct therapy

SIL assessment

Step 05:

PROFESSIONAL CLINICAL REPORTING

🚀 OTs translate these observations into a rigorous, professional report

🚀 Designed to meet NDIS requirements, providing the high-level clinical evidence needed to advocate for appropriate funding and supports

🚀
